Are you a compassionate and dedicated individual with a passion for making a positive impact in the lives of others?
We are currently seeking a Rough Sleeper Navigator to join our team in South East London.

Requirements for the Rough Sleeper Navigator:

  • Previous experience working with individuals experiencing homelessness or vulnerable populations, preferably in a support or outreach capacity.
  • Knowledge of local services, resources, and systems related to housing, healthcare, benefits, and social services in the SE6 area.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with individuals from diverse backgrounds.
  • Strong organizational skills and the ability to manage caseloads effectively, prioritize tasks, and work independently.
  • Compassion, empathy, and a non-judgmental approach to working with individuals experiencing homelessness.
  • Flexibility to work evenings, weekends, or irregular hours as needed for outreach and engagement activities.
  • Valid driver's license and access to a vehicle preferred.

Responsibilities (Social Care):

  • Provide high-quality homeless prevention, support, and advice to all rough sleepers in Lewisham.

  • Conduct assessments for rough sleepers presenting to the Council to determine owed duties and alternative housing options.

  • Act as a key worker for allocated rough sleepers, ensuring Personal Housing Plans are implemented and progress towards permanent housing is achieved.

  • Develop specialist knowledge and skills to deliver a flexible and responsive housing needs service.

  • Engage rough sleepers through outreach and partnerships with relevant services in the borough.

  • Maintain clear and accurate casework and data records, reporting on performance as required.

  • Signpost rough sleepers to local connection areas and support them in maximizing income through benefit applications and attending appointments.
