Salary: £34/h Weekdays

Job Description: Overview of Main Responsibilities of Team Leader Post:

  • To provide day-to-day management support of the multi-disciplinary team in delivering community care to service users.
  • To work closely with the Team Manager and other senior workers to develop the Community Mental Health Team.
  • To establish strong working relationship with the in-patient Service, Hillingdon HTT, Hillingdon Psychiatric Liaison Team and CNWL
  • To monitor workload and ensure fair distribution according to the various team roles
  • To attend monthly interface meetings and contribute to improving local care pathways.
  • To regularly undertake delegated managerial responsibilities and duties, and to deputise for the Team Manager in his/her absence
  • To undertake complaints and serious incident investigations in accordance with the Trust's Policies.
  • To offer regular structured supervision to staff within the team, as well as providing informal support
  • To undertake regular caseload scrutiny for categories such as discharge, step down or step up care
  • To participate with the Team Manager in staff appraisals.
  • To maintain clear, accurate and up-to-date recording of client work.
  • To work according to the principles of equal opportunities in all dealings with patients, staff, other professionals and the general public
  • To undertake regular audit of client case notes to ensure high standards of delivery of care are achieved and maintained

What you'll need as a Registered CPN:

  • NMC registration and educated to degree level
  • Experience in assessment
  • 6 months of paid patient facing UK nursing experience from within the past 3 years
  • Up-to-date clinical best practice knowledge
  • The right to work in the UK
