The Role:

The Reablement Project Officers will carryout reviews within the Reablement Service, this is to clear a backlog of client reviews where clients have been receiving a service for longer than 6 weeks. To note these roles are referred to internally as Social Worker Officers. This role will work with residents to ensure that we have accurate data for them on our client-based system (Liquid Logic). The role will also ensure that we get their feedback on the services they are receiving and seek feedback as to how services can be improved.

Social Work Officers will be required to make contact with residents, families and providers by phone and email ascertaining that the records we hold are correct and where changes need to be made ensuring that our systems are updated to reflect the changes made and the feedback we have received.

The Candidate:

5 GCSEs or equivalent including English and Maths (C and above).
A relevant Professional Qualification.
Relevant experience working in Reablement or as a Social care Officer in Adult's Social Care.
