Search are currently looking for Band 5 & 6 Rapid Response Nurses to work within the NHS in the Kent area (Gravesend).
Ideally they would like someone full time however, adhoc shifts are available and they are willing to be quite flexible.
As a Rapid Response Nurse, you'll be part of our valued team at Gravesend.
Duties:
Manage referrals and caseload, for a 7 day per week Rapid Response pathway.
Assist the Team Leader in running a team, deputising for the Team Leader in their absence.
Actively work as a member of the Rapid Response Pathway to provide high quality nursing care to clients within a community setting, preventing unnecessary hospital admissions and supporting hospital discharges.
Be responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
Implement, monitor and maintain high standards of care delivery at all times.
Care for patients and their families in the way that you believe is best.
Deliver our high service standards, sharing best practice, and actively improving the way we work.
