We are working alongside Devon to recruit an advanced practitioner within the Specialist Placement Team. You will provide leadership, support, and guidance at an operational level to enable the Local Authority to meet its responsibilities in relation to the assessment and care and support planning for people living outside of Devon with complex needs. Working in partnership with key agencies, you will ensure timely and effective assessment, personalised support planning, and care management, promoting integrated working and safeguarding adults at risk. Your role will be pivotal in ensuring teams and individuals understand and apply legislation, policies, and procedures to deliver high-quality services that optimise opportunities for individuals to manage their support.

The minimum requirements for this role are:

• Social Work England registration
• Social Work Qualification
