Location: Southend-on-Sea

Salary: £36,648 - £42,403 per annum

About Us: Our Leaving Care team is committed to providing high-quality support and guidance to care leavers, ensuring they have the tools and resources needed for a successful transition to independent living.

Key Responsibilities:

  • Work closely with young adults leaving care, assessing their needs and aspirations.
  • Develop and implement comprehensive pathway plans, promoting independence and well-being.
  • Provide practical support, advice, and advocacy to care leavers on education, employment, housing, and health matters.
  • Collaborate with external agencies to create a network of support for care leavers.
  • Maintain accurate and up-to-date records, ensuring compliance with statutory requirements.

About You:

  • Qualified Social Worker registered with Social Work England.
  • Experience in working with care leavers, understanding the challenges they face.
  • Strong knowledge of relevant legislation and policies pertaining to leaving care services.
  • Excellent communication skills and the ability to build positive relationships.
  • A commitment to empowering young people to achieve their full potential.
