Practice Manager | Older People and Disabilities Team | London Borough of Newham

We are looking for a Practice Manager with current management experience to join a busy front door Social Work team. This team is a mixture of Older People and Phsyical Disabilities. The Social Worker will based in the Newham, with the possibility of hybrid work.

Key Responsibilities /requirements:

  • Responsible for the direct line management, supervision and approval of Reablement plans for up of up to seven Reablement staff (including SW’s /Reablement officers /Social Care Officers)
  • Responsible for case direction and recording case directions
  • Responsible for performance management
  • Reports to the Reablement Team Management
  • Part of the Reablement management team, and able to deputise for the TM if necessary
  • Attend Integrated Discharge Hub calls if required
  • Able to work in a fast-paced environment
  • Good understanding of hospital to home (D2A) pathway

To be considered, you must have a Social Work qualification (Diploma in Social Work, Social Work Degree or equivalent) as well as at least 3 years post qualified experience working as a Qualified Social Worker within a similar team.
