Title: Lead Practitioner - Mental Health Team for Older Adults Location: Slough Salary: Up to £50,500 per annum

Do you have extensive experience as a Social Worker and a profound commitment to enhancing the mental health and well-being of older adults? Join us at Slough Council as the Lead Practitioner for our Mental Health Team dedicated to older people and drive positive change within our community.

Responsibilities:

Lead and supervise a team of social workers, ensuring the delivery of high-quality services in alignment with best practices and organizational objectives. Oversee the assessment process for older adults experiencing mental health challenges, ensuring thorough evaluations and personalized care plans are developed collaboratively with service users and their families. Set an example by providing therapeutic interventions and support to older adults, promoting independence, dignity, and an improved quality of life. Act as a liaison between the team and external agencies, fostering collaborative relationships to facilitate seamless access to services and resources for older adults with complex needs. Advocate for the rights and interests of older adults within the community, ensuring their voices are heard in decision-making processes and their needs are championed. Maintain accurate and up-to-date records, adhering to statutory requirements and council policies, while also providing support and guidance to team members in record-keeping practices. Facilitate team meetings, case conferences, and professional development activities to enhance the skills and knowledge of team members, promoting a culture of continuous learning and improvement. Requirements:

Qualified Social Worker registered with Social Work England, with significant post-qualification experience in mental health services, particularly with older adults. In-depth understanding of relevant legislation and policies governing social work practice, including the Mental Capacity Act, Care Act, and safeguarding procedures. Proven leadership and management skills, with the ability to inspire and motivate a multidisciplinary team to achieve shared objectives. Exceptional communication and interpersonal skills, with the ability to engage effectively with service users, families, colleagues, and external stakeholders. Commitment to promoting equality, diversity, and inclusion in service delivery, ensuring services are accessible and responsive to the diverse needs of older adults. Full UK driving license and access to a vehicle for travel within the community.
