Band 8a Service Manager - Operations

East Sussex

£220 to £235 per day

Cobalt PS are recruiting for a Band 8a Service manager to work within the Transfer of Care Hub department.

This will be a cross site role full time with the emphasis on clinically led, managerially enable services will be at the forefront of senior leadership within the operational culture

The Service Manager will provide leadership under the direction of the Transfer of Care Hub Manager and the Deputy Chief Operating Officer, they will analyse and evaluate complex key quality data, monitor, and act on trends and prepare reports for divisional meetings.

NHS Experience preferred

Duties:

  • Have extensive knowledge of current processes and discharge pathways to support the safe discharge of Discharge Ready patients across the system.
  • Develop, implement, and maintain high quality, safe and sustainable clinical services.
  • Improve access to services in the Division for the maximum benefit of patients by reducing waiting times and length of stay, modernising referral and other processes and supporting patient choice.
  • Support the Divisions with Length of Stay, education and training support around discharge pathways and assist with complex case management.
  • Work closely with the TOCH System Manager to make sure that KPI's are being achieved in relation to discharge metrics and that things are escalated as required in a timely manner.
  • Support the TOCH System Manager in Multi Agency Discharge Events (MADE) if required.
  • Will actively participate in improving the service and what the service offers to the divisions and support future developments.
  • Work with key system partners around escalation of complex patients.
  • Always making sure that the patient, carers, and the family voice are at the heart of discharge planning.
  • Work with our System Partners such as Local Authority ASC teams, Housing teams, Homeless service, and our Voluntary Sector to make sure that that these services are kept up to date with service change and support required.
  • Support the discharge check service in relation to themes which occur, feedback from patients/carers and relatives, and speaking to divisions where learning needs to be shared.
