Band 6 Occupational Therapist

Based at Perry Tress Care Centre (north Birmingham)

Role:

Assess new patient admissions, where possible jointly with physio

Identify short term goals, achievable within 2 weeks.

Devise a treatment and discharge plan with physio in accordance with Discharge 2 Assess model.

Liaise with patients and relatives about discharge plans.

Attend daily MDT tracker meetings

Complete necessary access visit assessments

Prescribe and order equipment

Refer to community therapy- Early intervention community team

Devise plan care plan for requesting package of care

Refer to social worker if required

Mental capacity assessment

Liaising with out of area local authority and ordering equipment and community therapy.

Support Band 5 with caseload, report to Band 7 with any complex cases

Ability to work cross sites - car driver essential
