Position Overview:

We are seeking an experienced and compassionate Adult Senior Practitioner Social Worker to join our dynamic Neighborhood Team. In this role, you will take a lead role in providing expert social work support to adults facing complex challenges, including mental health issues, disabilities, and social isolation. You will work closely with individuals, families, and community partners to empower adults to overcome barriers and thrive in their communities.

Key Responsibilities:

  • Lead the delivery of social work services for adults within the Neighborhood Team, ensuring high-quality, person-centered support.
  • Conduct comprehensive assessments of adults' needs, strengths, and goals, and develop individualized support plans in collaboration with service users and their families.
  • Provide expert guidance and supervision to social work staff, supporting their professional development and growth.
  • Coordinate multi-agency support services to address the complex needs of adults, including health, housing, employment, and social inclusion.
  • Advocate for the rights and interests of adults, ensuring their voices are heard and respected in decision-making processes.
  • Maintain accurate records, adhere to relevant policies and procedures, and contribute to service evaluation and improvement initiatives.

Qualifications and Skills:

  • Degree in Social Work, with current registration as a Social Worker.
  • Extensive experience in adult social work practice, including significant experience working with individuals facing complex needs.
  • Strong leadership abilities, with a demonstrated capacity to mentor and support colleagues.
  • Excellent assessment, intervention, and case management skills, with a focus on empowerment and resilience-building.
  • Effective communication and interpersonal skills, with the ability to build rapport and establish collaborative relationships..
  • Ability to work autonomously and as part of a team, with flexibility to adapt to changing priorities.
  • Knowledge of relevant legislation, policies, and best practices within adult social work.
