Mental Health Social Worker - Working for the NHS Trust

Requirements:

• Social Work England Registered
• Extensive Mental Health Social Work Experience
• Completing Mental Health Capacity Acts
• Enhanced DBS

This London Borough work in partnership with Oxleas NHS Foundation Trust provides a social work service for residents within fully integrated multi-disciplinary teams. They are looking to recruit an enthusiastic and dynamic Social Worker to join the Locality Adult Mental Health Team based near Woolwich.

As a Social Worker based within a multidisciplinary team you will be working with adults over 18 living with mental health conditions, undertaking Care Act Assessments and Care Plan reviews and act as a care coordinator. High priority is given to working together with service users, families and carers to enhance the independence and quality of life of people with mental health conditions.

Oxleas NHS Foundation Trust is a well- regarded Health Trust rated 'Good' in our last CQC inspection. The service is currently arranged into Primary Care Plus, which works with GP practices to maintain patient care within primary care and to aid the smooth transition between primary and secondary care,
