Leaving Care Personal Advisor

Service care Solution are currently recruiting for a Leaving Care Personal Advisor in Cambridgeshire

As a Leaving Care Personal Advisor you would be supporting Care Leavers aged 16-25 as they transition into adulthood. This is a Hybrid position.

Main Responsibilities

As a Leaving Care Personal Advisor, you will be responsible for:

  • Direct work and intervention supporting preparation for independence
  • Assessment, planning and review (Pathway Plans)
  • Working with partner agencies in DWP, Housing, Health and Education to achieve the best outcomes for care leavers
  • To assess the needs of young people in order to determine the level of advice, support and assistance they will require in making the transition to independence.

Requirements:

  • NVQ Level 3 in relevant area
  • Experience working with Children in Care
  • Enhanced DBS
