Job Opportunity: Adult Social Worker - Discharge to Assess Team

Location: Barrow in Furness, Cumbria

We are seeking an experienced Adult Social Worker to join our Discharge to Assess Team in Cumbria. As a crucial member of this team, you will play a primary role in preventing or delaying admissions into formal support services, ensuring individuals receive timely and appropriate care in alignment with the Care Act and Continuing Health Care guidelines.

Responsibilities:

  • Utilise expertise in discharge to assess processes to facilitate safe and timely discharges from acute and community hospitals.
  • Work collaboratively across the health and social care system and with third sector and voluntary groups to enhance prevention interventions.
  • Conduct short-term interventions and assessments using a strengths-based approach to ensure individuals are safe and supported.

Requirements:

  • Qualified Social Worker with relevant experience in discharge to assess, care act, and continuing health care.
  • Social work England registered
  • DBS on Update service.
  • Ability to work effectively within a multidisciplinary team and collaborate with stakeholders from diverse backgrounds.
  • Strong communication and interpersonal skills.
  • Commitment to promoting the well-being and independence of individuals.

Work Arrangement: The ideal candidate will be based preferably for four days a week in the hospital and one day from home. However, for the right candidate, we can consider a minimum of three days based in the hospital and two days working from home.

Location Considerations: Candidates must live close to Barrow in Furness or willing to travel, as accommodation cannot be provided.
