The Role: Discharge Co-ordinator

Working closely with NHS hospital staff, you’ll help to facilitate the flow and discharge of patients from hospitals into our community teams.

On a day-to-day basis, you’ll:

  • Oversee holistic clinical assessments for patients and potential patients, determining suitability for transition into a community setting, including our virtual ward and early supported discharge pathways
  • Co-ordinate discharge plans, identifying and resolving any internal delays
  • Identify care needs and provide support and advise as appropriate
  • Carry out face-to-face reviews of patients’ medical histories
  • Liaise closely with the Trust MDT and deliver real-time documentation.
  • ?As a key member of our multi-disciplinary team, you’ll help us to empower our patients in their choice to be treated at home.

About You

NMC registered, with at least three years’ experience, you’ll have the confidence to work autonomously in the community and share our commitment to delivering quality patient care. Compassionate and collaborative, you’ll bring with you strong communication skills and the ability to build effective relationships with patients, carers and colleagues alike.

You’ll also need:

  • The right to work in the UK without any form of sponsorship
  • A driver’s licence and access to a car, this is essential as it’s a mobile role
  • Knowledge of relevant and current legal frameworks and duties relating to hospital discharge and legislation.
