About the role

We have an exciting opportunity to join our talented team as a Dementia Adviser. You will have the rewarding experience of providing information, guidance and support to people affected by dementia in identifying their needs, to help maintain independence, improving sense of well-being, and putting them in more control of their lives.

This role will provide dementia knowledge and expertise within one of four locality Welfare Support Teams as part of the Integrated Care Across Northamptonshire. (ICAN) Ageing Well.

As a Dementia Adviser, you will:
  • Be the ‘go to’ person for all that is dementia related. 
  • Work closely with health, adult social care and third sector colleagues assessing, signposting, and providing information and guidance to support people affected by dementia.
  • Build robust working relationships and develop networking opportunities with a range of local partners and community organisations. 
  • Create pathways with partner organisations who work in the field of dementia, ensuring seamless, holistic consistent support and empowering individuals to make informed choices.  
  • Provide dementia support face-to-face, by telephone and online.
  • Work closely with the Age Well team, Memory Assessment Service and Older Peoples Mental Health Team and be able to facilitate our range of face to face and virtual groups as needed.

About you

We are looking for an exceptional individual, with an understanding of memory loss, dementia and the needs of those affected with these conditions. 

You will have:
  • The ability to assess client needs
  • An understanding of the need for client confidentiality 
  • Excellent non-judgmental communication skills
  • Strong organisational and time management skills
  • Good IT skills and experience working with databases
  • Have demonstrable experience.
  • An empathetic approach with a commitment to equal opportunity
  • The ability to travel independently across Northamptonshire.

As a community-based worker, you will be required to attend a range of meetings, appointments and venues as well as working from home. A DBS disclosure will be required.
