Community Nurse - Band 5 

Islington

Start Date: ASAP

Duration: Ongoing

Pay: £22/£24 per hour

Type: Full time or Part time (Min 3 days) 

What: Community Nurse (Band 5)

Main duties of the job

  • Delivering comprehensive nursing care to patients in their homes, covering wound care, medication administration, and chronic disease management
  • Assessing patient needs and crafting personalized care plans tailored to their unique circumstances.
  • Empowering patients and their families with knowledge and strategies to manage their health effectively.
  • Collaborating seamlessly with other healthcare professionals, including GPs, social workers, and occupational therapists
  • Contributing to the development and implementation of innovative community nursing initiatives
  • Keeping patient records organized and up to date to ensure seamless care coordination

To excel in this role, you'll bring:

  • Registered Nurse (RN) status with valid Nursing and Midwifery Council (NMC) registration
  • Proven experience as a Community Nurse or in a relevant clinical setting
  • Exceptional communication and interpersonal skills to build trust and rapport with patients.
  • The versatility to work independently and collaboratively as part of a team.
  • Impeccable organizational and time management skills to navigate a busy schedule.
  • A valid driving license for convenient patient visits
