Main duties of the job
- Delivering comprehensive nursing care to patients in their homes, covering wound care, medication administration, and chronic disease management
- Assessing patient needs and crafting personalized care plans tailored to their unique circumstances.
- Empowering patients and their families with knowledge and strategies to manage their health effectively.
- Collaborating seamlessly with other healthcare professionals, including GPs, social workers, and occupational therapists
- Contributing to the development and implementation of innovative community nursing initiatives
- Keeping patient records organized and up to date to ensure seamless care coordination
To excel in this role, you'll bring:
- Registered Nurse (RN) status with valid Nursing and Midwifery Council (NMC) registration
- Proven experience as a Community Nurse or in a relevant clinical setting
- Exceptional communication and interpersonal skills to build trust and rapport with patients.
- The versatility to work independently and collaboratively as part of a team.
- Impeccable organizational and time management skills to navigate a busy schedule.
- A valid driving license for convenient patient visits
