Community Mental Health Nurse
We are seeking highly motivated and compassionate Community Mental Health Nurses all across London. In this role, you will provide exceptional care and support to individuals with mental health conditions in the community setting. You will work closely with a multidisciplinary team to assess, plan, and deliver effective treatment plans and therapies, ensuring the overall well-being of your patients.
Key Responsibilities:

  • Conduct comprehensive mental health assessments and develop personalised care plans.
  • Administer and monitor medication regimes, ensuring compliance and managing any side effects.
  • Provide therapeutic interventions and support to individuals and their families, promoting recovery and resilience.
  • Deliver evidence-based interventions, including cognitive-behavioral therapy and psychoeducation.
  • Collaborate with community agencies and organisations to facilitate seamless care transitions.
  • Maintain accurate and up-to-date patient documentation, adhering to legal and ethical guidelines.

What You'll Need:

  • Valid NMC Pin and registration as a Mental Health Nurse.
  • Right to work in the UK.
  • PMVA training.
