Eden Brown Synergy has an exciting new opportunity for a Care Manager (Supported Living) in Milton Keynes. (Ofsted/CQC - Young People 16+)

Purpose of the role:

  • The Care Manager (supported living) would be required to create individualized care plans for service users.
  • The Care Manager will ensure all healthcare providers involved are up to date with case information and collaborate with service user families, friends, and caretakers to effectively share information and keep accurate service user records.
  • The Care Manager will link service users to community programs and entitlements, such as housing services and self-management programme

Essential requirements for this role:

  • a NVQ level 5 in leadership and management, or working towards completing -
  • Thorough understanding of Ofsted and the regulations and legislations that set the standards within a supported accommodation (2 years' experience preferred Ofsted/CQC)
  • The Care Manager (supported living) would have the ability to showcase leadership and management skills
  • Ability to multitask in a dynamic environment and have excellent negotiation skills.

Care Manager (Supported living) Responsibilities:

Young People

  • Contribute to the assessment of individual young people's needs and circumstances in line with Ofsted new Quality Standard.
  • Oversee the role of the Supported Accommodation Manager, Team Leader and support worker.
  • Provide effective oversight of the Supported Accommodation service.
  • Contribute to the planning, implementation, and evaluation of therapeutic programmes to enable individual young people to manage their behaviour
  • Prepare, implement, and evaluate agreed therapeutic activities
  • Review children/young people's individual needs and circumstances in line with statutory and departmental requirements
  • Provide a framework for the management of young people's behaviour
  • Support young people and, to some extent, their families to address issues which affect their health and social well-being
  • Promote young people's social and emotional development
  • Promote young people's sensory and intellectual development
  • Observe and assess the development and behaviour of children/young people
  • Represent young people's interests when they are not able to do so themselves
  • Establish and maintain relationships with parents/carers
  • Support young people experiencing a change in their care requirements and provision
  • Enable young people to maintain their personal hygiene and appearance

Partnership Working

  • Promote effective, professional communication and relationships with partners
  • Act as an advocate to ensure the best possible outcome for the young person
  • Establish and sustain working relationships with others

Staff Development

  • Promote and facilitate supervision, probation appraisal processes
  • Actively participate in personal supervision and appraisal
  • Promote and participate in training and staff development opportunities
  • Ensure the development and effectiveness of work teams

General

  • Carry out organisational objectives in line with relevant legislation and Ofsted Quality Standard.
  • Implement the development / service plan for the supported accommodation service.
  • Comply with administrative and financial procedures.
