Are you looking for a rewarding opportunity in the care sector? Join this well established provider of complex care in the community as a Care Coordinator for complex care.

We are looking for a dedicated individual to work Monday-Friday, 9-5, with on-call responsibilities. The on-call schedule is 1 in 3, planned 12 weeks in advance, so you will know when you'll be on duty. On-call shifts are either Friday night through Monday morning or Tuesday through Thursday, with three mid-week days required each month either as a block or separately at your discretion.

Your responsibilities will include coordinating with the Support Workers to get their availability, creating rotas and emergency shift cover. The salary for this role is £22k, with additional on-call compensation of approximately £4500-5000 per year. On-call shifts are from home, while office-based work is during regular business hours.
