Care Co-ordinator - Administrator

A leading care home in Addlestone is seeking and experience administrator to join the team. The ideal candidate will have previous administrative experience, excellent communication skills and be extremely organised, this is a full time role - Monday to Friday 9am - 5.30pm. No previous experience needed - full training will be given.

Daily duties will include -

Care review

  • Tasks: Use the Care review document to undertake the review and ensure it is signed by client when completed and upload on Oneplan.
  • Material: The care review document template to be provided.

Care Plan

  • Tasks: update existing or write up new Care Plan if non-existing, ensure is signed by client once completed and upload on Oneplan.
  • Material: Use the existing winsor Care Plan template

Spot check:

  • Call the client or their representative to notify them that you will be undertaking a spot check but do not notify the carers.
  • Carry out the spot check.

Speak with client to get a feedback on the service rendered.

  • Identify training needs or good practice.
  • Complete the Spot check paper work and upload it on Oneplan.

Supervision:

  • Arrange a suitable time with staff for supervision using Zoom giving them 1 week notice at very least.
  • Make a note of what was observed during spot check so as to add to the list of items to discuss.
  • Carry out supervision and upload on Oneplan.
  • Book the carer on training with a trainer if learning needs arise.

Staff File:

  • Check that there are no gaps in employment records on the applications form and where gaps occur, call the staff to get the information and fill in the additional employment history gaps form and counter sign that you spoke to the staff. (I’ll provide the additional gaps in employment history form)

Check that references are on file but if none chase both Professional and character references using the existing forms. If no referee details on application form, call the carer to get the details and update the form.

  • Send out references, hand over to me to follow up and upload on Oneplan.
  • Check that DBS in place and carried out by winsor otherwise conduct a DBS using existing winsor platform and handover to me to follow up and Upload on Oneplan once received.
  • Check that the carer has a work permit, if not in place call them to provide their work permit, check their identity and notify me if there’s a concern or are not complying.
  • Book the carer for refresher training with a trainer if none is completed in the last 3 months

Team Meetings -

  • To send out invitations to carers.
  • To obtain agenda from me and send it to carers ahead of the meeting while welcoming carers to tell us anything they want discussed.
  • To make contributions or feedback during the meeting base on observation from field supervisory activities.
  • To take minutes of the meeting.

Audit -

  • Carry out Medication Audit.
  • Carry out Care Notes Audit (25% each month, until 100% for 4th month).
  • Identify poor or good practices.
  • Discuss Identified practices during supervision.

Telephone monitoring questionnaire:

  • Carry out Telephone monitoring.
  • Identify concerns or compliments.
  • Discuss during supervision.
  • Update Manager on the outcomes of feedback.
