Adult's Social Worker

Team: Transitions Team

Location: Sheffield

Start Date: ASAP

Contract: On going

Hours: 37 and HYBRID WORKING

Rate: £30 per hour

An Adults Social Worker is required for the Transitions Team in Sheffield.

Applicants must have previous experience working with challenging client groups and be available for an immediate start date.

Responsibilities:

• Managing a case load and completing assessments, support plans and reviews with adults
• Completing Court of Protection documents, best interests and safeguarding investigations.
• Assess risk, explore suitable placements (Supported Living, residential and housing) alongside health professionals.

Experience:

• Must be experienced to deal with all aspects of the task and be able to hit the ground running
• Must be IT literate
• Liquid Logic experience desirable but training can be provided.

• DIPSW or equivalent qualification in Social Work
• Social Work England registration
• Post qualification experience
• Enhanced and in date DBS certificate.
• Excellent communication skills and be able to make good decisions based on information available.
• One year minimum post qualified experience in Adults Social Work
• Can work alone and within/contribute to a team/multi-agency set ups.
• Possess knowledge of service needs.
• Competent IT skills and be able to navigate electronic file/recording systems.
• Can write clear, concise, and robust assessment/reports/files.
• Have a flexible approach to work and working hours when the need arises
