Our Client, Sefton Council, is seeking an Adults Social Worker to join their Transitions Team.

Responsibilities of this role:

  • Utilising strength based approaches to empower individuals and support customers to maximise their potential.
  • Working in partnership to assess an individual's needs, support plan and help to arrange a variety of services and support to meet needs; utilising approaches that empower and support customers choice and control.
  • Meeting the challenges of both personalisation and working within significant financial constraints to develop creative support plans that deliver personalised outcomes and cost effective solutions.
  • Working in partnership to keep people safe and reduce risks. Depending on the role you may need skills and experience to undertake safeguarding investigations and develop and implement protection plans; and to assess risk and implement risk management plans.
  • Mental Capacity Assessments with individuals and where appropriate make best interest decisions
  • Good report writing and record keeping skills.
  • In the Development and Reviewing of services there will be a specific focus on working in partnership with colleagues across all Care Management Teams, Working Age Adults Contracts and Commissioning and a range of provider organisations to achieve 'value for money' through robust assessment and analysis.
  • You will be fully supported in this work by your manager, the wider management team and the experienced safeguarding and risk managers who will offer some joint work as well as being available for consultation.
  • In Transitions in addition to the above you would have the opportunity and skills to work with young people aged 14-25 years; having an understanding of child development issues and the ability to form relationships with young people and their families.
  • Additionally a basic knowledge of child safeguarding thresholds, Children and families Act and education and health care plans. All to contribute to a smooth transition into adulthood and adult service.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have at least 2 Years post qualified experience.
