Adults Social Worker - Liverpool - Community Mental Health Team

Liquid Personnel is currently recruiting for experienced and Qualified Adult's Social Workers to join a busy Community Mental Health Team based in Liverpool City Council.

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.

You will manage and monitor an allocated workload within individual, group priorities and Council policies and comply with the statutory obligations of the Group.

Your main responsibilities as a Social Worker:

• To make appropriate assessment and act as care navigator for the individuals presenting with eligible social care needs within appropriate time limits and in collaboration with health and other agencies.
• To plan relevant responses to meet individual needs in collaboration with carers, advocates, and representatives in consultation with health and other agencies.
• To ensure the delivery within financial resources available of services to meet assessed needs and outcomes.
• To maintain an effective system for coordination, monitoring and reviewing support plans and service standards, to identify gaps in service or unmet need and advise line manager and commissioners.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK license
• Post-qualified experience
• Degree level or equivalent in Social Work
