Our Client, Liverpool City Council, is seeking an Adults Hospital Social Worker to join their Team.

Fantastic payrate of £43.78 per hour!

Responsibilities of this role:

  • Conduct comprehensive assessments of adult patients' physical, emotional, and social needs to develop a deep understanding of their individual circumstances.
  • Coordinate and facilitate safe and timely discharge plans.
  • Provide advocacy and support to patients and their families.
  • Conduct risk assessments and implement safeguarding measures to ensure the well-being and safety of vulnerable adults during the discharge process.
  • Respond to urgent requests to visit anywhere in the borough.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must has at least 2 Years Post Qualifying experience.
