Adults Social Worker for Intermediate Care

We are currently seeking 5 experienced Qualified Social Workers to join our dynamic team in Sutton.
Our service operates within an integrated framework, collaborating closely with Adult Social Care and NHS services to support adults and older people in the community. You will play a crucial role in our Urgent Response service, aimed at preventing hospital admissions, as well as providing vital Rehab services.

Requirements (Adults Social Worker for Intermediate Care):

  • 2+ years post-qualifying experience in the UK social care sector.
  • Preferably experienced in physical disabilities, hospital discharge, or community neuro settings.
  • Knowledge of relevant legislation
  • Familiarity with MOSAIC
  • Willingness to conduct essential face-to-face visits with provided PPE.
  • Enhanced DBS check and Social Work England registration required.

Responsibilities (Social Care):

  • Conduct comprehensive assessments of service users' needs, ensuring a holistic understanding of their circumstances.
  • Conduct home visits
  • Implement and oversee safeguarding measures
  • Conduct Carers Assessments
  • Provide coverage for duty shifts as required, responding promptly to urgent situations.
  • Perform Mental Capacity Assessments
  • Actively participate in Multi-Disciplinary Meetings
