We are seeking an experienced Intermediate Care Social Worker to join our dedicated team in Bury. This role is crucial in preventing unnecessary hospital admissions and facilitating quicker discharges by providing the right support during patient recovery. The position is based in a residential care home with 36 beds, catering to younger and elderly adults with sensory impairments, dementia, and mental health conditions.

Day to Day of the role:

  • Conducting strengths-based assessments to improve the independence, rights, and risks of the people we serve.
  • Collaborating with Nurses, Occupational Therapists, Physiotherapists, and Carers to promote rehabilitation and recovery.
  • Providing community outreach and rehabilitation within people’s own homes.
  • Ensuring that patients receive the appropriate level of support during their recovery to prevent unnecessary hospital admissions.
  • Facilitating quicker discharges from hospital care.
  • Participating in regular team meetings, staff one-to-ones, and peer reflective supervisions.
  • Engaging in continuing professional development and training.


Required Skills & Qualifications:

  • 5 years post-qualifying experience in social work.
  • Previous experience in intermediate care or hospital settings.
  • Supervisor experience and the ability to be a practice educator.
  • Good knowledge of key legislation applicable to the role.
  • A commitment to embedding strengths-based practices in their approach.
  • A caring, motivated, and enthusiastic attitude towards improving patient outcomes.
