Adult Social Worker Transitions Team 16-25 - Windsor & Maidenhead

In your role as a Social Worker, you will be responsible for executing a wide range of statutory duties under the Care Act, Mental Capacity Act, and other relevant laws and policies. Your role will be performed with a focus on strengths, striving for high-quality outcomes, and a commitment to continuous improvement in your practice. You will work collaboratively with residents, caregivers, community organisations, and other key professionals to promote independence.

You will understand the significant impact that a well-functioning team can have on individuals requiring care and support, their caregivers, the council, and the entire adult social care department. This understanding will guide your social work practice, where you will prioritise delaying and preventing the need for social care intervention. You will engage in strengths-based conversations and adopt a relationship-based, person-centred approach to maximise the use of informal support systems and enable individuals to live their lives as they wish.

As a vital part of a multidisciplinary team, which includes social workers, occupational therapists, case managers, and support workers/advisers, you will work in a fast-paced environment at the heart of the Council’s corporate customer centre to deliver positive outcomes for residents with adult social care needs.

In your role supporting young people transitioning into adulthood, you will work within a team that ensures both the needs and opportunities to enhance the independence and status of young adults are addressed. You will recognise the novelty of this transition for families and will nurture and positively encourage them throughout the assessment process. The team serves two distinct groups of young people. One group includes young people diagnosed with Global Learning Disabilities, who can receive your support and additional support from an integrated team of Psychiatry, Psychology, Speech and Language, Physiotherapy, Occupational Therapy, and LD nurses as needed.

For young people without a diagnosed Learning Disability, you will assist them in preparing for adulthood, even in the absence of other health-related professionals who are not part of the integrated partnership offer.
