It’s the people that make the difference. We are looking for a dynamic individual to join our CCG Transformation team. We cover end to end pathway redesign from primary care, through to community services (including the voluntary sector) and acute services. We pride ourselves on doing our best for our population and making a positive difference.
We are looking to recruit a Band 4 Transformation Assistant who will provide high quality administrative and project support, using their initiative to maintain the effective delivery of key projects. This will be achieved through the effective use of information, data, knowledge and technology systems to improve, inform and support a portfolio of projects and service improvement initiatives
If you want to work alongside communities to transform services and gain experience across a wide range of commissioning areas, then we would like you to apply.
This is a fast-paced environment where you will be immersed in diverse challenges that will stretch your skills and offer development to achieve your personal and professional goals in a supportive team environment. A collaborative style, a good sense of humour and a desire to build successful relationships is essential.
You will work directly for NHS Fareham and Gosport Clinical Commissioning Group (CCG) and NHS South Eastern Hampshire Clinical Commissioning Group (CCG) which covers a population of circa 420,000 citizens. In addition you will also work across our Integrated Care Partnership to deliver innovation, across the Portsmouth and South East Hampshire system.
Ensuring patients receive the care they need, in the right place and at the right time is the top priority for the Hampshire and Isle of Wight Partnership of Clinical Commissioning Groups (the Partnership).
