Transformation Administrator

We cover end to end pathway redesign from primary care, through to community services (including the voluntary sector) and acute services. We are looking to recruit a Band 4 Transformation Assistant who will provide high quality administrative and project support, using their initiative to maintain the effective delivery of key projects. This will be achieved through the effective use of information, data, knowledge and technology systems to improve, inform and support a portfolio of projects and service improvement initiatives

You will work directly for NHS Fareham and Gosport Clinical Commissioning Group (CCG) and NHS South Eastern Hampshire Clinical Commissioning Group (CCG) which covers a population of circa 420,000 citizens. In addition you will also work across our Integrated Care Partnership to deliver innovation, across the Portsmouth and South East Hampshire system.

Ensuring patients receive the care they need, in the right place and at the right time is the top priority for the Hampshire and Isle of Wight Partnership of Clinical Commissioning Groups (the Partnership).

The Partnership serves a population of more than 1 million people and manages a budget of £1.4 billion. It is made up of five clinical commissioning groups from Fareham and Gosport and South Eastern Hampshire, North East Hampshire and Farnham, North Hampshire and the Isle of Wight.
