Now is a very exciting time to join the South West Division of Southern Health NHS Foundation Trust as we embark on a period of expansion within our existing teams and move toward a model of working as an ‘Academy of Community Health Care’.

The Academy model provides structured and accessible career pathways clinically and non-clinically, from band 3 all the way to 8b for the right people. We are committed to finding the right people for the roles, and then supporting them as individuals to achieve their full potential.

We are recruiting to multiple posts, to work alongside the local Primary Care Networks in Avon Valley, Lymington and New Milton, and Totton and Hythe. There is potential for this service to develop further across the South West Division, which also includes Romsey and Eastleigh. Successful candidates would be allocated a home base, and as a dynamic and flexible team may be asked to cover other bases as required from time to time.

These new post holders will have the opportunity to work across the three branches of The Academy, in Community Nursing, Community Therapy and The Frailty Support Team. All of our community teams provide urgent-acute and planned services to our patients. Which means no two days are the same!

The existing team of three Consultant Practitioners in Frailty are keen to support a senior clinician with advanced practice skills to develop the requirements of Consultant Level Practice.

The successful person would be joining a strong team of clinical experts in frailty, and would be supported over a three year period to develop a portfolio of professional evidence to demonstrate their competence at consultant level.

The post holder could become responsible for the future leadership of a Frailty Support Team, utilising their existing skills in project management, service design and implementation. They will work clinically to support a team of frailty practitioners to deliver care and interventions within a set locality.

You would be supported to develop academically and clinically alongside the maintenance of your portfolio.
