CHS Healthcare / Carehome Selection pride ourselves on being an expanding, dynamic company that has been synonymous with being a trusted provider to the NHS since 1995. We hold a large number of contracts with the NHS and have an ever expanding portfolio of work, making us the largest independent Care Coordinator in the UK. Since 1995 we have built a reputation as a trusted patient focused service specialising in assisting with hospital discharges to suitable care homes or sourcing care packages for clients leaving hospital, whilst supporting families in the process.
About the role
You will be home based within the East Midlands, covering Leicestershire, South Staffordshire, Derbyshire and Nottinghamshire. You will receive referrals from hospital and community staff for individuals who require a care home or to have care within their own home. Along with your Business Manager you will be responsible for meeting new providers and selling our service to them to ensure our teams have the best chance of placing our clients. Networking will be required within the hospitals and community to increase our profile and build referral numbers. You will manage your team to support families though every step of the process, meeting clear timescales. This is a high-quality service that evaluates very well with patients and families and also reduces unnecessary delays in this process.
What sort of person are we looking for?
As Team Coordinator, you will be the ambassador for this service - you will need to build strong, positive links with hospital staff, social workers and the different agencies involved. You will need to be a dynamic self starter, able to work on your own initiative to seek out opportunities for growth. You will need to be passionate about the quality of the service and the difference it makes at a critical time in people`s lives. You will need the confidence, authority, credibility and charm to persuade others.
Managing a team of field-based advisers will be a significant part of the role. Building friendly relationships, coaching, supporting and rewarding them should be something that you really enjoy and ideally have had experience of doing in the past.
Our Team Coordinators come from a range of backgrounds, including nursing, social work, care home management and sales. We have employed office managers, sales and marketing team leaders, lawyers but typically they come from a health and social care background.
You will also need to be highly organised and IT literate. All information must be recorded and you will be responsible for reporting on data and accounting for the service performance at senior meetings. At times of high demand, you will need stamina and the ability to ensure that your team works well under pressure.
Many of our employees are of graduate calibre, most with 5 GCSE`s at A-C level and above. Ideally, we are looking for individuals who have pushed themselves gaining other relevant qualifications, for example B-TECH, NVQ`S or Higher Level Professional Qualifications.
Please include your mobile number on your CV as you will be sent a text if you are shortlisted to take part in a structured competency based telephone interview
