CHS Healthcare/Carehome Selection is an expanding company providing an independent tailor made service throughout the UK; we support patients and their families as they face important decisions about their ongoing care after leaving hospital. Since 1995 we have built a reputation as a trusted specialised and patient focused service finding suitable care home placements and care packages most appropriate for their loved one.
About the role
You will be based at Stoke Mandeville Hospital, Aylesbury HP21 and provide cover to other sites attached to the trust. You will receive referrals from hospital staff and social workers for individuals who are in hospital and need to move into a care home or to have care in their own homes. You will manage your team to support families though every step of the process, meeting clear timescales for ensuring the patient moves into a community placement or receives care in their own home. This is a high-quality service that reduces unnecessary delays in the discharge process and evaluates very well with patients and families.
What sort of person are we looking for?
As team leader, you will be the ambassador for this service - you will need to build strong, positive links with hospital staff, social workers and the different agencies involved. You will need to be passionate about the quality of the service and the difference it makes at a critical time in people`s lives. You will need the confidence, authority, credibility and charm to persuade others.
You will need to be highly organised and IT literate. All information must be recorded and you will be responsible for reporting on data and accounting for the service performance at senior meetings. At times of high demand, you will need stamina and the ability to ensure that your team works well under pressure. Attention to detail and the ability to think on your feet are essential to this role.
You will be managing a small team of both office and field based advisers. You will be responsible for coaching, supporting and rewarding your team and therefore experience of this will be required.
A healthcare background is an advantage; however, your management and interpersonal skills are more important to this role. Our Team Co-ordinators come from a range of backgrounds, including office managers, sales and marketing team leaders, lawyers, nursing and social care management.
Many of our employees are of graduate calibre, most with 5 GCSE`s at A-C level and above. Ideally we are looking for individuals who have pushed themselves gaining other relevant qualifications, for example B-TECH, NVQ`S or Higher Level Professional Qualifications.
