To support individuals in their own tenancies following a timetable of therapeutic tailored activities. Your role will be to have a genuine desire in their interests and hobbies. These may include shopping, cooking, baking, keeping their house clean and tidy, walking and everyday hobbies and interests. You will need to continue to explore and introduce new community based activities. You will need to provide support to maintain relationships with their families and other agencies. To assist the individual to plan their future, acknowledging their likes and dislikes, enabling them to reach their full potential and make decisions to fulfil their hopes and dreams. You will be supporting individuals to complete their normal daily living tasks. Administering and monitoring medication including side effects and effectively seeking advice if required. You will be recording daily notes and be part of planning, implementing and evaluating support plans. Effective MDT working will be required.
