I am looking for Support Workers to work in a rural service located in the village of Westonzoyland, approximately 4 miles from Bridgwater. The property is located adjacent to the site of the historical Battle of Sedgemoor of 1685. It is a long established home within the village and has developed strong local community links with local amenities being accessed daily including shop, butchers, pub, community hall and GP surgery.
Supported Living accommodation is provided in partnership with a Housing Association and is laid out over a single story. My Client provides accommodation for 7 people. There is also a self-contained ground floor flat adjacent to the main building currently providing single dwelling accommodation. Trained staff experienced in supporting individuals with profound multiple learning disabilities and complex behaviours provide a caring and supportive service according to the assessed needs of each person.
Person centred planning is central to the services. The plan starts with the individual, focusing on their everyday needs, risks and aspirations and results in achievable goals for both the individual and our staff to follow. These plans are reviewed regularly to ensure progress is monitored and that they continue to be relevant and meet changing needs. We take great care in overseeing the wider issues of general health and well-being, both in helping individuals to lead healthy lifestyles, and ensuring professional support and treatment is provided when necessary.
The look to enable individuals living at within the home to: -Take part in community life with the full rights and responsibility of citizenship - live as independently as possible with care and support individually tailored to meet that person's needs - make informed choices about all aspects of their lives - have valued relationships and develop a network of friends and acquaintances and be involved in key decisions, which affect their lives.
