Salary Range:£8.00 - £9.50 ph, negotiable & subject to experience
Term:Part-Time & Full-time, permanent

My client is a provider of residential and supported living care for people with Autism, mild/moderate Learning Disability and Mental Health needs. The organisation is looking to develop a team of dedicated support workers for its two Northwood-based services. Each of the services has a maximum of 3 clients and are located within close proximity to one another.

As a support worker, you will be responsible for supporting people with their needs and goals, both inside the home environment and out in the local community. Activities may include daily living skills, support with medication administration, personal care and travel escort whilst attending community-based leisure activities.

Job Description
*To provide support in accordance to an individual's support plan and ensuring that support plans are relevant and up to date.
*To ensure that all information is clearly communicated, recorded and monitored, and that the highest standard of care is maintained in accordance with policies, procedures and CQC Fundamental Standards.
*To provide practical assistance with personal tasks such as eating, drinking, meal preparation, laundry, cleaning and shopping, and to help maintain a high standard of personal hygiene.
*To administer prescribed medication in accordance to specific guidelines & support plan.
*To liaise with clients' families, friends and carers who feature in the person's life.
*To work independently with confidence and as part of a team to ensure the best possible service is provided.
*To preserve and respect the dignity and privacy of people and their families, and observe confidentiality.
The Candidate
*Must have a minimum of 12 months' experience of supporting clients with a Learning Disability & Mental Health conditions with complex needs.
*Knowledge of the needs and rights of our client group, and understanding the issues they may face.
*Must be approachable and attain a good level of spoken & written English.
*An NVQ in Health & Social Care level 2 or equivalent, or a willingness to work towards this qualification.
*Must be eligible to work in the United Kingdom.
*A willingness to work flexibly, covering early & late shifts, some nights & sleep-ins, weekends and bank holidays
The offer of a permanent post is conditional upon a clean Enhanced DBS record and successful completion of the required probationary period.
