Support Worker

Due to growth Transition Care are currently looking to recruit Support Workers to join established and new teams within our organization.

You will be supported within the role with onsite managers and out of hours on-call managers available 24/7 for advice and support.

Location: Sefton/Liverpool

Hours: 37.5 hours per week.

Salary: £7.83 per hour plus overtime

Job type: Full and Part Time Permanent

Holidays- 5.6 weeks paid holiday

DBS fee paid

Transition Care support individuals with Mental Health Needs, Learning Disabilities, Acquired Brian Injury and Physical Disabilities within our Supported Living Properties and community support to individuals in their own homes .

Experience within the care sector is an advantage but not a necessity as full training will be provided. The right values, beliefs and attitude is essential.

Job Requirements:

To support our service users to maximize their independent living skills. Providing prompts and support to them following Person Centered Support plans that include providing emotional support,self-care and wellbeing, attending appointments, medication, finances, day to day living skills such as cooking, cleaning, shopping and leisure activities., and 1-1 support to attend college, work opportunities and community groups.

You must be willing to undertake all training relevant to the role. The role will be subject to a six-month probation review.

You must also be flexible and willing to work shifts, morning, evening ,nights and weekends.
