Vantis Resourcing is recuiting a full time Support Manager, the role involves managing a supported living service for adults with learning disabilities, dual diagnosis of Learning Disabilities and mental health and a range of other needs including Autism and behaviours that might challenging.

The service has been developed to support individuals coming from a range of settings including long stay mental health hospitals and secure residential units. Our aim is to support the individuals back into the community and reduce the risk of hospital readmission.

Key Responsibilities of Manager manager:

You will be key to ensuring a smooth transition is in place working with families, professionals and empowering the individual to move in and be at the centre of this journey. Your role will include completing capacity, support assessments and transition plans for people, as well as liaising with key people involved to ensure a successful process. You will continue to improve the quality of Services, updating the people supported plans, risk assessments and completing person centred plans to promote independence and achieve outcomes.

Requirements of Support Manager:

Our Client will support the right candidate to become facilitators of Pro-act SCIP training and Active Support to truly encourage, involve and inspire the people we support to lead active and fulfilling lives, developing independence, confidence & being a valued member of their local community.

  • You will be a driver, with access to a car, as you will be required to work across sites and travel for training.
  • We are looking for candidates who can work flexibly to meet the needs of the services and participate in the On-Call rota.

  • NVQ 4 or above
