Support Coordinator (Stroke Survivors)

I am excited to be working with a large charity supporting individuals who have suffered from strokes who are currently recruiting for a temporary Support Coordinator to join their busy team covering Reading and Wokingham. 

Based mainly in the community, the successful candidate will responsible for: 

- Ensuring that the needs of Service Users and carers are identified and addressed thoughout their recovery. 
- To work collaboratively with a range of organisations, clinicans and other professionals to create meaningful networks, providing the best possible support to Service Users. 
- To recruit, develop and coordinatr volunteers to support the service, ensuring a wide range of accessible and innovative support options are available. 

Person Specification: 

- Educated to a minimum GCSE level or equivalent. 
- Experience of working within the Health & Social care industry. 
- QCF/NVQ Level 2/3 in Health & Social Care is desirable. 
- Ability to communicate with strok survivors and carers effectively. 
- Ability to set and review realistic goals and boundaries.
