Support Coordinator - Housing

My Supported Housing Client in the Maidstone area is looking for a Support Coordinator to join their team on a temp - perm basis. 

The role is:
- 37 hours per week
- Monday - Friday

And the following is essential:
- Use of Car
- Ability to work a rota to provide out of ours support
- Experience of working in a supporting/social; care environment for at least 2 years

You will be expected to provide a high quality support service to customers throughout the community via, assessments, support planning and meetings. Co-ordinating the appropriate support services which underpin tenancy/home sustainability, and you will need to comply with the KCC Commissioning Team contracts working to defined service specifications.
