Hywel Dda University Health Board have a fantastic opportunity for a pharmacist to work cross-sector in our friendly teams in Bronglais hospital (2 days / week) and across Ceredigion as part of the Medicines Management team supporting primary care (3 days / week). This post will enable you to deepen your knowledge and expertise, and broaden transferable skills under the direction of experienced colleagues. You will build on previous experience, developing greater autonomy and responsibility, whilst gaining an understanding of the whole patient journey.
In Bronglais Hospital you would be joining a team of secondary care pharmacists, pharmacy technicians and support staff who provide ward based care to admissions units, specialist medical, surgical and rehabilitation wards as well as specialities such as Acute Medicine, Surgery, Critical Care, Care of the Elderly, Women's and Children Health.
As part of the primary care team you will offer targeted support in areas such as patient safety and medicines optimisation. You will advise and influence various healthcare professionals, including GPs, nurses and prescribing clerks in GP surgeries. You will be supported by a friendly, welcoming team with a vast amount of expertise to help mentor you on your career path.
The post requires a highly motivated Pharmacist with excellent communication, interpersonal and time management skills. You will provide a high quality clinical pharmacy service, participate in training of healthcare professionals and contribute to the planning and delivery of the medicines optimisation strategy. As services move closer to home, increased communication and collaboration between the sectors will be key to a successful service. Pharmacists with cross-sector experience will support future delivery of integrated services, ensuring seamless care for our patients.
