We are looking for a person who wants to work within a motivated and enthusiastic team based at Breconshire War Memorial Hospital. The successful candidate should have experience of working in the community as the role is that of a Reablement Therapist working with local authority support workers to assist people in their own homes and communities to provide admission avoidance, discharge planning and working collaboratively with third sector and voluntary services.

The successful candidate will also be required to support the South East Community Therapy Team and demonstrate knowledge in equipment assessment including moving and handling.

You will need to have excellent communication and organisational skills, a flexible approach to work and negotiation skills to motivate patients and staff to deliver high standards of care.

The ability to speak Welsh is desirable and we welcome applications from Welsh and /or English speakers.
