Due to promotion of the post-holder an exciting opportunity has arisen for a band 6 Specialist Nurse at Humber Teaching NHS Foundation Trust based initially with the Hull West CMHT at the Waterloo Centre.
The community mental health services in Hull are undergoing further change as part of the broader mental health services transformation agenda and we envisage a much closer working relationship with the local Primary Care Networks to enable seamless delivery of care for people with mental health problems.
Both of the CMHTs in Hull work closely to provide a consistent approach particularly with regard to the management of incoming referrals and so the post-holder may be required to support the hospital discharge and liaison function as part of their role.
You would be part of a multi-disciplinary team comprising a range of professionals with access to professional support and supervision. As a member of the team you will be expected to contribute to a team approach to clinical decision making as well as managing a caseload of patients with complex and challenging mental health needs.
The successful applicant will be able to demonstrate relevant experience and post-qualification training appropriate to this post or be willing to work towards obtaining further a qualification e.g. Non-Medical Prescribing. You will be encouraged in your professional development and benefit from a robust staff appraisal process, managerial, practice and professional supervision.
You will also be expected to support the Team Leader and Clinical Lead to help shape and deliver integrated mental health services for clients with severe and complex mental health needs.
The successful applicant needs to have the ability to commute within the Trusts geographical area and access to an appropriate means of transport is essential.
