Currently Leeds City Council are recruiting for a Social Worker.
This is a 6 month contract within the Meanwood Neighbourhood Care Management team based at Rutland Lodge on Scott Hall Rd in Leeds.
Leeds are looking for a social worker to undertake initial assessments and reviews, organise discharges from community beds, and from the neighbourhood health team. To carry a caseload, including some long term complex work with young adults with disabilities. The worker will also be required to undertake duty on a rota basis.
The worker will need to be a registered social worker with the HCPC, be proficient in using information systems, and open to new ways of working as Leeds is moving to a strength based model of care.
