Delivering services for people living in Hampshire with physical disabilities.An opening has arisen for an experienced Qualified Social Worker to join our community teams, which are located over four Hub bases in Hampshire, Basingstoke, Havant, Totton & Romsey.As a qualified Social Worker, registered with SWE, you will need strong interpersonal and organisational skills. In this role you will support people with physical disabilities to gain greater independence to meet their outcomes.You will need to travel independently across the county to meet with individuals and carers to assess needs, to undertake strength based reviews using a person-centred approach designed to provide meaningful choice and promote wellbeing. Working in accordance with social care legislation and local government requirements, informed by the values and principles of social care; you will be supporting in the delivery of person centred, safe and effective social care service demonstrating excellent strength-based practice. We actively collaborate with other agencies to put people of Hampshire at the heart of decision-making. Working with partner agencies to promote independence and wellbeing.Our priority is to safeguard the most vulnerable and to be as efficient as possible, whilst utilising the opportunities afforded by closer working with partner agencies, working proactively to promote seamless transitions in care and independent safe living within the community. You will have excellent case management skills or relevanttransferable skills, as well as risk assessment skills. The ability to work in partnership with individuals to identify their care and support needs is vital to the role and will draw upon your excellent communication skills. Working across a wide geographical area, the teams will work in a mobile and flexible way, making best use currently of homeworking, HCC hubs and drop-ins. As such, you will need to be confident in working autonomously and willing to embrace full use of digital technology. You must be able to work autonomously, prioritise your work and respond flexibly to competing demands. An excellent working knowledge of legislation and current policies is required. Social Workers in the team are required to undertake various duties to include assessments under the Care Act 2014, mental capacity assessments,reviews of funded care and safeguarding work.Due to the nature of the role, the successful candidates will have a full driving license and access to a car.If you have a sound understanding of adult social care and a commitment to delivering a high-quality service, we would love to hear from you.
