This is an exciting time to join the Hospital Discharge Team within the Perth & Kinross Health and Social Care Partnership. You will work with adults and carers who require an urgent, person centred response. You will be expected to give advice and practical help on the problems associated with daily living activities. The successful applicant will provide a range of social work and care management tasks to assist in supporting vulnerable people to return home from hospital. These include carrying out Outcome Focussed Assessments, the negotiation of flexible packages of care, and the monitoring and reviewing of individual care packages.

Responsibilities will also include statutory work under the Adults with Incapacity Act and the Adult Support and Protection Act.

The successful applicant will also have responsibility for providing a specialist resource within Adult Social Work through the development of knowledge and experience in care areas of practice and to carry responsibility for cases with a high level of need, the complexity of which social work experience, knowledge and skills are required.
