The teams are generic adults, dealing with older persons, LD and safeguarding.
To apply for this role you must be a passionate social worker who strives to empower the service users and help their families. You also must have pervious experience of working in an adults team, hold a current HCPC and have a DBS.
• Carrying out Assessments
• Obtaining multi-disciplinary assessment of need
• Duty work
• Care management
• Designing intensive care packages
• Carrying a caseload of service users
• Make recommendations for short-term care, intermittent care and permanent residential care where appropriate
• Self-directed support/personal budgets
• Court of Protection application including Finance and Welfare
• Safeguarding enquiries
