In this role you will empower people to help them take control of their health and wellbeing. You will meet with people referred into the service on a one-to-one basis, in GP surgeries, community venues, or carrying out home visits where appropriate. During the Coronavirus outbreak we are using digital technology and the telephone to support patients.
You will provide non-judgmental support, giving time for patients to focus on what matters to me. You will connect people with non-medical support, including statutory services, community groups and activities to improve their wellbeing and tackle social isolation and meet their non-medical health and wellbeing needs. You will take a holistic approach, working in partnership with local GP practice requirements, community groups and multi-disciplinary teams.
