Senior Support Worker

Care First Recruitment LTD currently require supported housing officers within various London Boroughs.

The main purpose of the position will be to organise and plan effective community based support interventions for service users. To have lead responsibility for Mental health Resource service management, well-being and support planning of service users. To carry out all duties in line with organisational policies and procedures.

Key Aims:

- Enable residents to avoid, and in particular to reduce the need for, prolonged hospital admissions and long term dependency on mental health services.

-To promote recovery and prevent social exclusion among service users with mental health issues.

-Improve each service user’s quality of life including promoting and maintaining independent living.

-Provide relevant support in accordance to the service user’s needs via the risk and support planning process.

-Supporting service users to develop skills required for independent living and address their broader needs by enabling access to other support and advice services.

-Encourage and promote a multi-agency approach of working to ensure the needs of the service users are met to a high quality which will include working in partnership with a range of care and support agencies within a support planning framework.

-Work in partnership with other agencies to provide support for people with mental health needs.

Key Responsibilites:

-To be responsible for the support and well-being of all service users, using person-centred planning a key-work system to enable them to attain and sustain a tenancy, enjoy a good quality of life and avoid repeat homelessness.

-To assess referrals, formulate and review support plans, risk management plans and move-on plans with all service users.

-To allocate key-workers, coordinate and supervise Recovery Worker & Employment Support worker

-To work in partnership with outside agencies involved in the care and support of service users.

-To deputise for the team leader in their absence

-Assist the manager to plan, negotiate and manage budgets by operating within agreed financial policies and procedures and to make best use of property, equipment and information technology.

You must have experience within this role and this experience would need to have been within the last 18 months

All successful candidates will require an enhanced DBS check as well as 5 years checkable work history
