Reed are currently seeking a Senior Practitoner for an Adults integrated multidisciplinary team in Westminster. 

You must have a degree in Social Care or equivalent and be HCPC Registered. 

Job Duties:
ssist Team Managers in managing the integrated multidisciplinary team.
Responsible for the case management and CPA co-ordination of complex cases.
Act as a mentor for others on complex cases including safeguarding.
Promote the values of independence, rights, choice and inclusion as stated in
• ‘Valuing People Now’
• The Care Act 2014
• MC Act
• All policies and practices relating to Adults with Learning Disabilities.

Ensure that assessment, case management services and specialist clinical services are available for Westminster residents who meet the eligibility criteria for the Westminster Learning Disability Partnership.
Participate in the Duty management rota for the service
Participate in the allocation of work within the multidisciplinary team
Supervise members of the team as outlined in the departmental guidelines.
Carry out staff appraisals in accordance with departmental and clinical governance guidelines.
Ensure that work undertaken by the team is recorded accurately and in a timely manner both on paper and electronic files and on the service database.
Take part in working parties and other service development activities.
